Please read the following agreements carefully.  They contain policies that are important for your massage therapy session.  If you have any questions or concerns regarding these policies, please do not hesitate to ask for clarification.

As my client, you agree to the following:

1. I will pay all my session fees at the time I receive my massage.  If I am using insurance coverage, I am responsible for billing the insurance company directly.

2. If I am going to be late for my appointment, I will call as soon as possible and I understand that my time might be shortened as a result.

3. All the information on my Intake Form is correct and up to date.  It is my responsibility as the client to update my therapist about any pertinent changes in my health during future appointments.

4. This massage is for stress reduction and relief of muscular tension or spasm and is not a substitute for medical examination or treatment.

5. The therapist does not diagnose, treat or prescribe for any illness, ailment or disease.  While the therapist may assist in relief of physical or emotional symptoms, I understand that the therapist will not “fix” me. 

6. This is a non-sexual massage.  Any misconduct or inappropriate behavior will result in the immediate termination of the massage with full payment due.

7. I am always in control of my body during my session and will feel free to comment on the comfort or discomfort of strokes and pressure being used.  I can say “stop” at any time during the session, for any reason.

8. If I have a cold, flu, sore throat, stomach virus, poison oak, skin rash, anything contagious – I will reschedule my appointment.

9. I will be not be under the influence of drugs or alcohol at the time of my session. 

10. I will have showered on the same day as my session.

11. Payment by check or cash is due before the massage.  If my check bounces, I agree to pay a $30 service fee, as well as any additional fines the therapist might incur as a result.

12. Twenty-four (24) hours notice of cancellation is required, otherwise I will be required to pay $30 for either a late cancellation or a missed appointment.

As your Massage Therapist, I agree to the following:


1. Client is provided with a competent and professional session each time they come for an appointment.  Therapist will provide services for which she or he is qualified (both physically and emotionally) and able to do.

2. Accurate records are kept and client’s medical history and chart will be reviewed before each session.

3. Client will be fully covered with a sheet (known as a drape) at all times and only the body part being worked on will be uncovered.  Genitals will never be exposed or massaged.

4. Provide a customized treatment to meet client’s needs and will refer to appropriate specialists when work is not in my scope of practice and/or not in the client’s best interest.

5. I respect all clients regardless of their age, gender, race, national origin, sexual orientation, religion, socio-economic status, body type, and political affiliation, state of health or personal habits.

6. Privacy and confidentiality are maintained at all times and all clients are treated with respect and dignity.
